The following is a list of steps and requirements including the procedure Frontier must follow prior to installing your High Speed Internet (HSI) service, liabilities regarding the service once it is installed, payments on the service, what to do if you are moving out of the HSI service availability area and discontinuing the service.

  1. Frontier must coordinate the testing of the HSI line prior to activating the system. The testing will:
    • Verify whether the HSI service will work on the premises.
    • Allow for the coordination of the HSI line maintenance.
    • Assist you in repairing or replacing the modem if required.

  2. HSI will not be responsible for any and all liability, costs, damages or causes of action which result from any use of the contracted services.

  3. Invoices must be paid within 30 days of receipt. If you have any questions regarding billing, you can call 1-888-977-JACK Monday through Friday from 8 AM to 9 PM and Saturday from 9 AM to 5 PM, or on Sunday from 10 AM to 7 PM.

  4. Service payments will be absolute and unconditional in all events and will not be subject to any set-off, defense, counterclaim, or recoupment, for any reason whatsoever. A late payment fee equal to the maximum amount allowable by law will be charged against any account for any payments of any amounts due hereunder that are not received within thirty (30) days of the date of the invoice.

  5. Customer shall pay when due, all charges and taxes (local, state, and federal) that may now or hereafter be imposed on the possession or use of the service, excluding, however, all taxes on or measured by Frontier income.

  6. A pro rata credit allowance will be made for an interruption in the HSI service of 24 hours or more.

  7. The service term will commence on the date of installation. The term will continue in full force and effect for twelve months from the date of installation. HSI must be in service for twelve months. If canceled prior to twelve months of service, the customer's account will be charged a penalty fee. This fee is equivalent to the cost of the modem (if not returned in working condition with 10 business days of the cancellation) plus $120.00.

  8. Customer shall provide Frontier with thirty (30) days prior written notice of its intention to terminate this service.

  9. If the customer moves from a HSI-capable area to another HSI-capable area and wishes to have HSI installed at the new location, the customer will be liable for any installation costs at the new location (unless the special promo offer is in effect in the new area) plus $25.00.
